All six New Jersey starting situations

JNY-NJ-01

Approaching school exit; adult services not started

DDD eligibility, the NJCAT, school transition, Medicaid, and vocational rehabilitation are connected but separate actions.

Read the step-by-step guides
  1. Choose the correct DDD application and submit it with the records you have.
  2. Prepare for the NJCAT, complete it with the people who know the person, and save the tier record.
  3. Coordinate the final school year so DDD, support coordination, the ISP, and DVRS do not start after exit.

JNY-NJ-02

DDD eligible; Medicaid or program status unresolved

DDD functional eligibility, Medicaid eligibility, Supports Program enrollment, Community Care Program status, level of care, and service authorization are different decisions.

Read the step-by-step guides
  1. Verify active Medicaid and use the DDD help route for a denial or termination.
  2. Get a written explanation of the person’s current DDD program, waiting-list, and level-of-care status.
  3. Select a Support Coordination Agency and confirm the assigned Support Coordinator.

JNY-NJ-03

Enrolled; planning and authorization underway

The person-centered plan, NJCAT tier, annual budget, service definition, rate, authorization, and actual service start must line up.

Read the step-by-step guides
  1. Turn the person’s goals and support needs into an approved, operational service plan.
  2. Match the NJCAT tier and annual budget to the exact authorized service, unit, and current program rate.
  3. Confirm that enrollment, planning, authorization, provider or employee onboarding, and start scheduling are all complete.

JNY-NJ-04

Choosing providers or self-directed services

Provider discovery, direct capacity verification, model choice, service authorization, and rate implementation require separate checks.

Read the step-by-step guides
  1. Use Provider Search, then contact each provider to verify the facts that the directory cannot establish.
  2. Compare provider-managed services with Agency with Choice and Vendor Fiscal/Employer Agent self-direction.
  3. Convert a provider or worker choice into a verified, authorized service start.

JNY-NJ-05

Eligibility, Medicaid, or support decision is denied or uncertain

DDD eligibility appeals, Medicaid fair hearings, planning disputes, and parallel employment, housing, benefits, and decision-support routes have different owners.

Read the step-by-step guides
  1. Use the current written DDD notice to request the correct eligibility appeal.
  2. Use the current Medicaid notice and DDD troubleshooting route without losing the notice-specific hearing deadline.
  3. Keep other adult-life pathways moving while a DDD or Medicaid issue is reviewed.

JNY-NJ-06

Urgent need, crisis, abuse concern, or service breakdown

Immediate emergency response, behavioral crisis stabilization, abuse reporting, service correction, and reassessment are separate actions.

Read the step-by-step guides
  1. Use 911, 988, or Trinitas CARES according to the immediate situation.
  2. Document the breakdown, contact the responsible service owner, and use the emergency or reporting route when safety or abuse is involved.
  3. Document changed needs and request the assessment and plan changes required to respond.
